Community Psychiatric Nurse

We are offering you the opportunity to work in an innovative Community Mental Health Team (CMHT) within the Psychological Medicine and Older Adults Operational Directorate.The CMHT is multi-disciplinary, with all professionals represented and participating in general mental health work, as well as their own specialisms. The team serves people over 65 years and their families who live in the Borough of Lewisham in South East London with both functional and organic mental health issues.

In the role of Care Coordinator, we will be encouraging you to work as an autonomous practitioner, with our client group to help maintain and support their mental wellbeing in the community and offering support to their carers. Working for the Mental Health of Older Adult and Dementia, ensures personal and career development by attending collaborative staff development forums and Nursing Councils, allowing staff to network with senior management of both the Directorate and Trust level, supporting staff with further education opportunities, the opportunity to be involved in Research happening across the Trust South London and Maudsley NHS Foundation Trust is one of the largest providers of mental health services in the country. It provides services locally across Croydon, Lambeth, Lewisham and Southwark as well as regionally, nationally and internationally.

The South London Mental Health and Community Partnership (SLP) is a collaboration between Oxleas NHS Foundation Trust, South London and Maudsley NHS Foundation Trust, and South West London and St George’s Mental Health NHS Trust, delivering mental health services to a population of more than three million people. The partnership brings together clinical expertise, experience and innovation, aiming to improve quality, use resources most effectively, and deliver best practice consistently to all patients.
